For your convenience, and to minimize wait time, parent(s)/guardian(s) of students in grades 1-12, who are new to the district, must contact the Board of Education Office to schedule an appointment for registration.
All kindergarten registration will continue to be performed at East Elementary, located at 506 N. Aukerman St., Eaton, OH 45320. For additional information regarding kindergarten registration, please call 456-5173.
Required documentation for registration:
- Birth Certificate
- Proof of residency
- Immunization records
- Custody papers/protection orders (if applicable)
- Previous school records (will be requested by Eaton Schools if parents are unable to provide them)
The following forms must also be filled out during registration:
- Emergency Medical Authorization Form (for registration after August 23, 2010)
- Student Health History (for registration after August 23, 2010)
- Home Language Survey
- Support Services Verification
The above listed forms may be filled out prior to registration, by downloading the forms posted under Downloads.
Students are not required to attend the registration process. After registration has been completed, a time will be scheduled for you and your child to visit his/her designated building and/or schedule classes.
